Check all that apply. ■ Anonymous ■ Identify how the community can continue to monitor their success- create tools for them, a survey they can utilize to track their progress.
● Long term Goal- Develop a process survey for assessment to cover the following: ○ Examples:
■ What worked, what went well, what could be better? ■ Have you attended these sessions? Check all that apply. ■ Community Needs Assessment once per year
● Gather residents to complete this by partnering with stakeholders and attending events. Keeping them engaged, what questions are they interested in? What do they want to learn? What is the most significant change you have noticed? Are you planning to adopt the information learned?
